PAF A-5c Fantan flying over Pishawar

Posted on 12 March 2012 by Tea Server

PAF Twin ship A-5 c Fantan Formation flying over Pishawar.

The A-5C Fantan is export designation of the Chinese designed J-5
(Attack aircraft 5), which is a twin-jet attack aircraft, derived from
J-6/Mig-19 earlier produced in the People’s Republic of China.

According to one report, at least 210 Fantan-As were in
service with the PLA Air Force by 1979, serving with tactical attack
squadrons of the People’s Liberation Army.

A 1980 report declared that the aircraft had been built in
relatively large numbers, and it is known to serve also in an air
defence role with the air arms of the PLA Navy. The total number in
Chinese service in now probably in the region of 500-800.

Deliveries of an initial batch of 52 export A-5′s to the
Pakistan Air Force began in February 1983 and have been completed.

The configuration of the PAF’s A-5′s differs considerably
from that of their Chinese counterparts, several design features having
been introduced at Pakistan’s request to increase the aircraft’s strike
range and enable it to carry several additional types of weapons.

Why We Write?

Posted on 12 March 2012 by Tea Server

There are hundreds of pages, thousands of words and millions of letters being invested on writting. People share their thought, their emotions, their perspective on every issue below the sun and beyond it. Every thing is discussed from issues related to minor problems such as using an hammer on an inclined floor to complex issue pertaining to Quantum mechanics, from domestic issues to international mayhem, and from animal rights to human rights. People giving out there opinions and thoughts which eventually makes the world a better place to live in.
Writing brings change! (Source: rehmalda.com)
Why do people write? Why do they want to invest their time on scribbling down words on a piece of paper when there are more important things to do? The answer is to this question is simple. Every man and woman has an hidden feeling, a secret beast or angel, a magic wand, a desire, a hope, a will to change and prosper, a desire for a better tomorrow, a blessed world for the next generation and global peace in today’s world.

There are people who write because they get money from writing. People who generate money from writing are of two types. Those who -through their words- want to change the world for a better tomorrow. People like Habib Jalib, Emmeline Pankhurst, Malcom X, Noam Chomsky and Eqbal Ahmed invested their intellect to fight authority. Authorities which denounce human freedom and curtail rights of common man. These aforementioned people in every part of history have stood up against the norms of time and vehemently opposed injustice. Injustice at home or abroad. Injustice against men, women, Muslims, Jews, Arab, non-Arab. They have raised their voices against evil dictatorships and usurpers. The annals of history has enshrined their names for all times here after.

There are also people who sell their words for petty prices. People such as the German philosopher Heidieger who supported Hitler’s Fascism in the third Reichs. The Pakistani Lawyer Abdul Hafeez Sheikh who supported all dictators from the creation of Pakistan till present. Selling words gave life to death systems and led to a societal meltdown.
The real intellect always opposed such writing which fuelled hatred in the society. They were sent to jails for their words, and for their actions against the oppressor. But history has shown, oppression always faces defeat whether it is in Germany, Italy, Libya, Tunisia, Iraq, Iran, Pakistan, Soviet Union or Burma. In future we’ll also witness the  collapse of governments in Bahrain, Syria and even in China and Saudia Arabia. Such defeats have given life to the words of the intellects. Words have risen from ashes, like the phoenix of the Greek mythology, like the voice of Rose Parker against white domination, like Nelson Mandela in South Africa, like Gandhi and Jinnah in South Asia, like Karl Marx against the capitalist exploitation and like the writing of Aleksendr Solzhenitsyn against war.
This is why we write!

Posted on 11 March 2012 by Tea Server

Prominent Pakistani television artist Tahira Wasti has died in Karachi on Sunday, She was 68. She was a wife of Rizwan Wasti,  a famous Pakistani television and film actor and presenter, who died last year.Tahira Wasti has undeniably been one of the most professional and
adept actresses that the television screen has seen in at least a couple
of decades. There were speculations that Laila was suffering from a life-threatening disease and was undergoing treatment. No television viewer, who has seen the dramas produced in the late
eighties and nineties, can forget the unforgettable performances given
by Tahira Wasti in every role that she was a part of.

Her roles were mostly supporting but that didn t inhibit her from showing her true potential through her skills as an actress. Her earliest dramas include  Afshan ,  Jangloos .  Daldal ,  Piyaas
and long play  Heer Waris Shah , aired on PTV and  Kashkol , which was
screened on former channel STN. Although she took a break from acting for a few years, she has
reemerged with a revamped look in a number of productions both on PTV as
well as private channels. Her recent performances have come in the dramas like  Harjai ,  Dil
Diya Dehleez ,  Maamta  and  Doraha . She has also acted in a tele film
by the name of  Uss Ki Biwi . She was wonderful lady and actress. Her work will be remembered forever. May her soul rest in peace.!

Aur karo Pepsi 65 ki

Posted on 11 March 2012 by Tea Server

Pepsi is again of Rs.65. Pepsi
new electronic ad is very bad. Pepsi is now of Rs.65 but they showed this
discount in very despicable way. Sms come and people run and why they run it’s
showed in end of ad. How a bridal runs on her first night through window.Thats
not a Pakistani Culture. Pepsi new ad is not depicting Pakistani Culture; they
are spoiling youth. How a person runs when he is in barber shop and barber is
cutting his hair. Pepsi Pakistani advertising is biggest failure. In contrast
to new India ad Change the game of Pepsi is excellent ad as compared to
Pakistani ad. Pepsi Pakistan should hire new advertising agency and this ad
should be banned.

Asia Cup: Important for Pakistan

Posted on 11 March 2012 by Tea Server

Written for Khelo Pakistan .

Asia Cup starts from the 11th March.  According to most people this tournament never gets the deserved status it should have and thats kind of true as its always organized at the wrong time. According to Geoffery Boycott no one cares about Asia Cup not even the Asian teams. But is that really true for every participating team.

India have just come back from a long tour of Australia and with IPL just around the corner they would have liked more rest. Srilanka were in South Africa and with a gap of few days traveled to Australia for the Tri-Nation series and played their last match just 2 days ago. Bangladesh are the underdogs and don’t have much chances.

But is this isn’t just another tournament for Pakistan who have played non stop cricket for the last five months, If at all this tournament is important for a team, its Pakistan. Winning the Test Series against England comprehensively, but losing the ODI and T20 series, Pakistani Team’s future depends on this tournament.

This tournament for sure will be the last chance for Misbah-ul-Haq to show his capability in the shorter format of the game firstly as a captain who can attack more and secondly a batsman who can score quickly. After losing just one series as a captain Misbah Ul haq is under fire both from the general public and media. He will try his best to prove the critics wrong.

Asia Cup is also the first test of Dav Whatmore who took over as Pakistan’s coach just a week ago. Lots has been said about him, his experience coaching the Srilankan and Bangladesh Cricket Team, his strictness and his good reputation, but coaching Pakistan is totally different and will take the best out of him especially because Mohsin Khan was doing a good job.

This is also the first team selected by the new selection committee lead by Iqbal Qasim. Team selection tells the mind set Pakistan team has. They have tried not to experiment with the team retaining the same bowling squad which played against England and have included Nasir Jamshed and Sarfraz Ahmed instead of Imran Farhat and Shoaib Malik who were labelled as “Sifarishi”.

If Pakistan are able to perform well in this tournament, the stability under Misbah Ul Haq will continue for a considerable time as the next two series Pakistan will play are against Bangladesh and Srilanka. But If they don’t perform well for sure it will bring an end to a few cricketing careers in the ODI format, Pakistan will have no choice except to change the captaincy and bring in new players at the cost of Misbah Ul Haq or even Younis Khan.

They still have a choice though even If they don’t win the Asia Cup.  18th of March, when Pakistan play India. The emotional nation we are, winning or losing the Cup won’t matter if Pakistan is able to beat India, taking the revenge of Mohali 2011. So yes, Pakistan has lots to play for in Asia Cup.

Effects Of Media On Our Brain

Posted on 10 March 2012 by Tea Server

Today I’m gonna tell you the affects of media on the human mind. Now, I’m no psychologist who has been studying about this, I’m just a normal guy who has been experiencing something weird. Whenever you switch on the news channel, you probably hear the word ‘terrorist’ at least once a day. Most of the times when you hear it you see a picture of some
Arab guy or specifically a Muslim, usually in the photo the person has a beard or has covered his entire face with a cloth and is holding a very famously known terrorist used weapon, yes, you’re right, the AK-47. They also sometimes wear a head band which says something like this:

Whenever we see this, our brain automatically points towards Muslims, because they are on the news so often and the word ‘terrorist’ is used for them so many times, that we now only see their faces. I think this is a really huge problem. There was once a time when people used to look for a taxi driver who had a beard, as they were known for being honest and truthful. But now, people have bad images in their brains towards people with a beard, people get a bad image when they hear the words Islam and Allah.

This is a picture of an American soldier along with a picture of an Arab soldier. Who looks like a terrorist to you?

Your brain will most probably point towards the Arab soldier. The media has made them look so bad and has made them look like the bad people but in real, the terrorists are the Americans, who are killing men, women and children by the thousands every year in Iraq, Palestine, Afghanistan and now, in Pakistan as well.

How to argue against PML N: A Beginner’s Guide

Posted on 10 March 2012 by Tea Server

If you are using any form ofSocial Media, you come across PTI Trolls or IMRANIs, as I like to call them,on a daily basis. Now in most cases when you come across them, they make emotionally charged arguments that require less than 2 minutes to be demolishedand broken down. They keep on going about the same things the PTI website tellsthem or worse sometimes they make arguments from the statements of their greatleader. Now thing is, it is annoying listening to their arguments which most ofthe time are pretty damn stupid. I mean they have no idea what a line ofargumentation even means, what they do is bring in random buzzwords and keepharping on about them. Having been through enough of these semi-moronicarguments, I decide to help the Imranis out by writing a few arguments forthem. I am laying out 3 decent arguments that can be made by anyone who hatesPML N and not look like an idiot while doing so.

Arguing the Laptop issue

The rookie mistake here is, whenan Imrani normally argues this, they start by quoting random figures they pullout of thin air. Once asked to substantiate them, they start finding sourcesand often end up finding sources from the Chief Minister’s own website. Nowthing is, you cannot attack someone who has the actual figures and facts on hiswebsite. That makes you look stupid and the other person look like a pioussaint. The next stupidity is starting to argue about the state of education inPunjab in general. This is a bad path to go down because firstly it’s notrelated to the Laptops and secondly every major International Institution heapspraises on Punjab for its quality of education. So bringing this in ispointless.

The correct way of arguing thisto raise questions like, so what to do with a laptop in an area that has no electricity and has no internet coverage? Are they for typing only cause I do not see any printers being given out?Why no internet access with them? Is the government trying to Wifi the wholePunjab? If there is no internet provision with the Laptop i.e. the governmentis not paying for it, then how does the government expect low income households to get internet? Also if the laptop isused how are the people supposed to print stuff? Is the government planning to give a subsidy on internet for laptops handed out by them? If Yes, how long would this subsidy last? Does the Punjab Governmentplan to give printers soon? In essence Laptop is like giving someone a CarEngine, its crucial but it requires other things to make proper use of it. So when you are going to argue its importance or need, use one the above mentioned questions to launch the argument as they are potent and cannot be shoved aside.

Arguing the Health care Issue

Most Imranis do not know how togo about this. They just normally watch a few clips of Dunya or Express TV andstart acting as if they know everything. In addition to this, they would alsosimply repeat their great leader’s words and call CM Punjab and Nawaz Sharif ‘Dengue Brothers’. Now thing is, calling names is childish and immature. This issomething that the Imranis and their great leader do not understand. What isworse is that using names like this in arguments make the whole argument weakand kind of pathetic.

The correct way of doing this is,by raising questions like, why does all of the health care attention only focuson Central Punjab? How come the state of health care institutions in northernPunjab is so bad? What about Southern Punjab? Why does the government keepgiving in to doctors and their demands when everyone knows that they are notright? If there is good governance, why isn’t there a proper website or apublic platform that lists all the medicines that are available through thegovernment? Who buys medicines on behalf of the government? Why is the Punjab Government randomly approving more medicalcolleges in the province when they could easily be expanding the existing ones? Why is money being wasted on projects that cannot be replicated across Punjab? Now see each one of these questions is a solid one, it requires a detailedanswer and in most cases that answer does not exist as yet. If these were beingused in an argument, it would genuinely make sense and have an impact, insteadof calling CM Punjab a ‘Dengue Brother’.

Arguing the Infrastructure Projects

Again in this regard, the Imranisor all critics in general do not really know what to do. They just attack thecosts of projects and what not, which is good to make a splash but it’spointless when used in an argument with people who have a brain. Attacks ofcost of a certain project or the idea that it is for personal benefit arestupid and lead to nothing. So instead of doing that, here are questions thatwould genuinely have an impact and create a robust argument.

How come most of the budget for Punjab’sdevelopment gets spent in Lahore? Is it the Government of Punjab or Governmentof Central Punjab, because the way things are it looks like the latter? Why doall infrastructure projects in the province keep getting tendered to the NLC  and FWO without any open tenders? Why is that a year before the elections, the PML Ngovernment has dug up nearly all major cities of Punjab? Were they sleeping for4 years that they did not realize that development projects had to be done? Whydid the Punjab Government cancel the Lahore Mass Transit System that was beinginitiated by the previous government even though it was a great system andcould have changed Lahore forever? Why is the new Bus service that is supposedto run on Ferozepur Road Lahore being based out of Thokar Niaz Beg which is no where near the Ferozepur Road? How comedevelopment in Punjab happens in concentric circles i.e. anything closer toLahore gets more developed while the further away the area is, the more ignoredit is?

The three key arguments I havelaid out here are genuine sensible arguments. These are questions that followsome line of logic and require genuine answers. The point is, if you are goingto attack someone and bash them, at least does it properly. Do not come up withidiotic arguments as they hurt you more because they make you look stupid. Soplease my personal request to anyone attacking the PML N, use one of thesearguments and stop making your hollow pathetic attempts at arguing them downemotions.

I am not writing a guide for PPP or PTI because that is just too easy. MQM scares me and ANP sells out too easy. Oh I do not consider PML Q as an actual party.

Breast Cancer | SKMH

Posted on 09 March 2012 by Tea Server

Breast Cancer

Dr. Neelam
Siddiqui

FRCP (G)
Consultant Medical Oncologist (Shaukat
Khanum Memorial
Cancer Hospital
& Research Centre)
Breast Cancer poses a serious health risk for women
throughout the world. More than one million women world wide are diagnosed with
breast cancer every year. It is estimated that 1 in 9 Pakistani women will
develop breast cancer at some stage of their life.  In developed countries there are national
cancer registries where every newly diagnosed cancer patient is registered.
This is very helpful to understand the epidemiology and causative factors of
cancer. In Pakistan
we do not have such a registry at national level. Several studies and reports
suggest that among Asian population, Pakistani women have the highest risk of
breast cancer (after non-Arab Israeli women).
Breast cancer is a disease which results from the interaction
of different environmental and inherited risk factors. With the development of
new technology by which DNA of cancer cells can be studied closely, our
understanding of the biology and genetics of breast cancer has greatly
improved. With this knowledge therapies for breast cancer are coming onto the market,
which have given a new lease of life to these patients. Since the early 1990’s
death rates from breast cancer have decreased by approximately 25% in the USA and Europe.  This has been possible mostly due to
screening mammography and continuously improving treatment strategies utilizing
chemotherapy, hormone therapy and more recently targeted therapy.
 While the prognosis
of breast cancer has improved over the years, the exact cause of breast cancer
still remains unknown.  However certain
risk factors have been identified. The following factors are known to increase
the chances of development of breast cancer.
Female gender and
increasing age:
As a
woman grows older her risk of developing breast cancer increases.
Hormonal factors: Early age of onset of menstruation and
late menopause are associated with an increased risk of breast cancer. Women
who do not have children or have their first child after the age of 30 are also
at an increased risk. Prolonged hormone therapy is an additional risk factor.
Benign breast disease: Women who have had non-cancerous lumps
in the breasts are also at higher risk of getting breast cancer compared to
those women who have never had any breast problems.
Diet and Obesity: New studies indicate that obesity
increases the chances of breast cancer and breast cancer related death.  Women who gain weight in adult life before
menopause are at increased risk of developing breast cancer. It has been
observed that high fat intake can increase the risk of breast cancer while high
vegetable consumption may give protection against breast cancer development.
Family history: This plays an important role in
breast cancer. Familial susceptibility to breast cancer accounts for
approximately 25 % of all breast cancer cases. Among the breast cancer genes
that have been identified, two are worth mentioning. These genes are called breast
cancer 1 (BRCA1) and breast cancer 2 (BRCA 2) genes.  Alterations in these genes are associated
with a significantly increased risk of breast and ovarian cancer. A woman who
has two or more first degree relatives with breast or ovarian cancer (especially
before the age of 50), may be carrying the BRCA1 or 2 gene.  These women could benefit from screening, risk
assessment, genetic counseling, gene testing and appropriate medical interventions.
The outcome and prognosis of breast cancer is directly linked
to the stage at which a woman is diagnosed with breast cancer.  It is difficult to prevent breast cancer, but
one can try to detect it early. In this context, all women of reproductive age
should conduct monthly breast self-examination while those over 40 should have
an annual clinical breast examination and an annual mammogram. Development of a
breast lump or skin and nipple changes are some common signs of breast cancer
and warrant an urgent visit to the doctor.
During October all over the world breast cancer awareness
campaigns are being conducted. In Pakistan we need to develop breast
cancer prevention and screening strategies. There is a dire need for creation
of more effective treatment which is easily available to all classes of
society. Aspiration towards excellence should be the goal of every physician. We
in the medical community need to step forward to fight against breast cancer by
establishing  a national breast cancer
control programme to reduce the risk of developing breast cancer. We should aim
to detect breast cancer as early as possible and to offer the best treatment
and support for our breast cancer patients.

Breast cancer signs and symptoms

Dr. Amina Khan
FACS
Consultant Surgical Oncologist
(Shaukat
Khanum Memorial
Cancer Hospital
& Research Centre)
Breast cancer is the most common malignancy
encountered by the woman of today. It is a disease process which is not
restricted to any age; it spares no race color or creed. According to
statistical estimates every 1 in 8 to 1 in 15 women will be diagnosed with
breast cancer during their lifetime.
Every woman’s risk is different and depends on a
combination of factors, some of which are alterable. Age, race, gender,
geographic location, family history, age of menarche and age of menopause are
factors that are non modifiable. However women should try and avoid risk
factors that are within their control; avoid late first birth of their child,
have more than one baby, breast feed all children, avoid hormone replacement
therapy, avoid smoking and alcohol use.
In contradiction to common misconceptions breast
cancer generally carries a good prognosis and is treatable in most cases. The
key to successful treatment is early detection and prompt medical attention.
The international cancer community has devised guidelines to help in this
regard – women are divided into 2 groups based on age – <40 and above 40
years.
* All women should learn proper breast self
examination and perform it regularly once every month.
* All women need a baseline mammogram at the age
of 40 years
* Follow up screening mammograms should be done
annually after the baseline study
* Any new finding on breast self examination or
mammography needs to be reported to a physician and investigated further
Important findings on
self examination that need further evaluation include

* A new area of thickening in the breast or a new
breast lump
* A lump in the axilla / arm pit area
* Nipple distortion
* Bloody nipple discharge
* Skin retraction or dimpling
* Fixation of skin to an underlying lump
* Skin redness
* Skin thickening especially around the nipple
area
* Feeling of warmth over an area of skin which is
already thickened and red
* Skin ulcer or blister that does not heal
* Skin rash over the breast especially around the
nipple with lesions that do not heal
* Pain in the breast that travels to the arm pit
and along the arm and neck

Treatment of Breast Cancer

Dr. Narjis Muzaffar
Diplomate American Board of Medical
Oncology
Consultant Medical Oncologist, (Shaukat Khanum Memorial
Cancer Hospital
& Research Centre)
There are different types of treatments available for
patients with breast cancer. In recent years, there has been on explosion of
new life saving treatments for this cancer. Treatment includes surgery and
radiation therapy which are localized treatments and chemotherapy and hormonal
therapy which are systemic treatments. Both chemotherapy and hormonal
therapy  go to many parts of the body, the
purpose of which is to kill any cancer cell that may have spread  from the breast.
The most recent addition to breast cancer treatment is
called  targeted therapy.
Clinical trials are also offered to patients which test new
drugs. Patients are encouraged to take part in these clinical trials which will
help not only in their treatment but also other women with breast cancer and
will help future generations. No one treatment plan fits every woman diagnosed
with breast cancer.  The choice of
treatment is determined by many factors including age, menopausal  status, stage of the tumour, hormone receptor
status and Her 2 Neu receptor status of the tumor.
Surgery is generally offered at the onset. The extent of
surgery will be determined by many factors such as the size of the tumor and
its location in the breast.  The patient’s
surgeon will best guide her in making the right decision for the type of
surgery. Surgery can be total removal of the breast, called mastectomy, or
removal of the lump only, called lumpectomy.
Breast reconstruction is the rebuilding of the breast after
mastectomy or lumpectomy.
This can be done at the time of the initial surgery or
later.
Many women decide not to have reconstruction and opt for a
prosthesis.
If the tumor is large or if the woman wants to conserve her
breast chemotherapy or hormonal therapy can be offered before surgery which can
shrink the tumor and  help the surgeon in
getting clear margins after a mastectomy or help in breast conservation.
The type of chemotherapy used can vary from patient to
patient. There are many highly effective treatment plans that can vary in
duration, number of drugs used and dosage. Results of clinical trials done in
thousands of women from all over the world help the medical oncologist on
deciding for a specific treatment for each individual.
The side effects of chemotherapy can be managed through
supportive care and lifestyle changes. Older women in otherwise good health can
also benefit from chemotherapy in the same ways as younger women.
Hormonal therapy blocks the ability of the female
hormones  estrogen and progesterone to
stimulate the growth of cancer cells. These drugs will only work if the woman’s
cancer cells show presence of these receptors.
Radiation therapy is limited to one area and is generally
well tolerated. Side effects of radiation are also limited to the area being
treated. It is a highly effective way of killing cancer cells in the breast or
chest wall that may persist after surgery.
Targeted therapies for example the drug Herceptin, target
specific characters of cancer cells such as proteins. These proteins allow
cancer cells to grow in an abnormal way. These drugs are generally well
tolerated.

At SKMCH&RC state of the art
treatment is offered to patients with breast cancer.
